Job Description:
Seeking a dedicated RN Patient Care Navigator for the Heart
Failure team to support patients through assessments, advocacy,
education, and personalized case management.
Acts as a liaison, referring patients to appropriate resources, and
collaborates with multidisciplinary teams to ensure seamless
care.
Conducts comprehensive health and psychosocial assessments,
develops care plans, and educates patients and families on
treatment options and community resources.
Requires a BSN, 3+ years RN experience (preferably in telemetry,
ICU, CVICU, or heart failure), Virginia RN licensure, and BLS
certification.
Work schedule is Monday-Friday, 8:00 a.m. to 4:30 p.m., with travel
between care sites.

Full-time, in-person role based in Falls Church, VA, with
relocation required prior to start.
