• Receive and manage medical records at the onsite office.
• Review and track medical records and invoices received via mail.
• Maintain remote access to required EHR systems.
• Perform remote data mining for quality gap closure directly in EHR systems.
• Conduct outreach to provider offices to request medical record data.
• Serve as a Subject Matter Expert (SME) for data collection and HEDIS inquiries from provider offices.
• Review and abstract medical record data into appropriate applications.
• Follow up with offices that have not complied with records requests or have sent incomplete documentation.
• Analyze chart data, evaluate data integrity and deficits, and document findings.
• Navigate multiple documentation systems to obtain medical record sections supportive of HEDIS/Star measures.
• Support hybrid chart chase processes by requesting records from provider offices as needed.
• Maintain knowledge of HEDIS/STAR standards and guidelines.
• Perform other related duties as assigned.Minimum Required Education, Experience & Skills
  • Licensed Professional/Vocational Nurse.
  • One or more years of provider outreach and/or EHR system navigation experience.
  • One or more years of HEDIS/STAR experience.
  • Two or more years of experience in data analysis/quality chart reviews, with the ability to review medical records and assess completeness.
  • Excellent oral and written communication skills.
  • Proficiency in Microsoft Office applications, including databases, word processing, and Excel spreadsheets.Preferred Education, Experience & Skills
    • Associate degree.
    • Undergraduate degree or equivalent experience.
    • Certified Professional Coder.
