Job Title: Nurse Practitioner Core Job Location: San Antonio, TX Duration: 6 Months Shift: Mon – Fri :8:30 am – 7:30 pm, Sa: 9 am – 5:30 pm, Su: 9 am – 5:30 pm. Every other weekend required. Position Summary: · The Family Nurse Practitioner (FNP) will work in collaboration with a dedicated clinical care team to provide evidence-based, holistic care in a retail clinic setting. · The FNP will independently manage a panel of patients aged 18 months and older, delivering comprehensive services including assessment, diagnosis, treatment, and health counseling. · This role is integral to driving improved patient outcomes and reports directly to the Senior Practice Manager. Key Responsibilities: · Patient-Centered Quality & Safety · Educate patients on health maintenance and wellness strategies. · Evaluate, diagnose, treat, and co-manage medical conditions to ensure high-quality care. · Accurately document all patient interactions using an electronic health record (EHR) system. · Assist in the development of Patient-Centered Practice Teams. · Provide timely responses to patient inquiries.
