You will be a trusted guide for families, a clinical expert for patients, and a valued member of our interdisciplinary team. Key Responsibilities Perform comprehensive assessments and ongoing evaluations Develop and implement individualized care plans Provide symptom management, medication administration, and comfort?focused interventions Educate families on disease progression, care techniques, and available resources Collaborate with physicians, social workers, chaplains, and hospice aides Maintain accurate, timely documentation Participate in on?call rotation as needed Offer emotional and spiritual support consistent with patient and family preferences Qualifications Active Registered Nurse (RN) license in Texas Minimum 1 year of nursing experience (hospice or home health preferred) Strong clinical assessment and communication skills Valid driver's license and reliable transportation A heart for service and a commitment to compassionate care What We Offer Mileage reimbursement Supportive leadership and a mission?driven culture Opportunities for professional development and continuing education
