This Home Health Physical Therapist role offers the opportunity to
practice at the top of your license in Marble Falls, Texas, a
vibrant community in the heart of the Texas Hill Country. Nestled
along Lake Marble Falls and just minutes from Lake LBJ, the area is
known for its striking granite outcrops, rolling hills, and
expansive water views. Residents enjoy boating, paddleboarding,
kayaking, fishing, and lakeside sunsets, along with miles of nearby
hiking and biking trails in local and state parks. The community
offers a relaxed, friendly small-town atmosphere, while still
providing convenient access to shopping, dining, and cultural
events, and is within driving distance of the Austin metro area.As
a Home Health Physical Therapist, you will provide one-on-one,
patient-focused care in the comfort of patients homes across Marble
Falls and surrounding Hill Country communities. Your caseload will
primarily include adult and geriatric patients recovering from
orthopedic surgery, falls, neurologic events, cardiopulmonary
conditions, and chronic disease exacerbations. Each day you will
complete functional evaluations, develop individualized plans of
care, and implement evidence-based interventions designed to
maximize safety, independence, and quality of life at home.A
typical day may include conducting initial assessments and
re-assessments, coordinating start-of-care and resumption-of-care
visits for new referrals, and performing follow-up treatments such
as gait and balance training, strengthening and endurance programs,
transfer training, stair and uneven-surface mobility, home safety
modifications, and pain management strategies. You will provide
extensive education to patients and caregivers on home exercise
programs, safe mobility techniques, fall prevention, and use of
assistive devices and durable medical equipment. You will also
collaborate closely with the broader interdisciplinary team,
including nursing, occupational therapy, speech therapy, social
work, and home health aides, to ensure cohesive and
patient-centered care plans.The home health service area typically
includes Marble Falls and nearby Hill Country neighborhoods,
allowing you to build long-term relationships with patients and
gain a strong sense of connection to the community you serve. Visit
volumes and patient ratios are designed to support quality,
one-on-one interactions, with scheduling that allows dedicated time
for documentation using an EMR platform optimized for mobile,
field-based work. You will regularly communicate with referring
providers and clinical leadership to update progress, adjust
treatment plans, and support timely transitions of care.This role
is ideal for a Physical Therapist who values clinical autonomy,
